MD4 EXCELLENCE AWARD
CRITERIA
- To be considered for this award, the nominee must be a Lion in good standing in a club in good standing from a district within Multiple District Four.
- The Lion must show great leadership ability and demonstrate that ability in thought and deed.
- A nominee shall have exhibited leadership by taking initiative with projects from their club and district.
- The Lion must have demonstrated dedication to the Lions organization and served as a role model for all Lions.
- Consideration for nomination must be given to lifetime achievements of the candidate as well as to the candidate’s Lionistic attitude.
PROCEDURES
- One(1) award for each 1,000 members in each District, or major portion thereof, as recorded on July 1 of the beginning of the fiscal year.
- A Lion will only receive the award one time. The award will be presented at the District’s annual convention by a current MD-4 District Governor on behalf of the multiple district.
- Nominations for this award may come from clubs or the district cabinet and shall be in writing. Each nomination will contain full justification for the award, showing that the nominee is the most deserving available and that the nominee meets the criteria for the honor.
- The district cabinet will review all nominations and select the recipient(s) at a regularly scheduled meeting of the cabinet. The selection shall require a majority of votes cast of the cabinet members present. The decision of the district cabinet shall be final. The name(s) of the district recipient(s) shall be submitted to the council of governors prior to the February council of governors meeting.
- The District Governor, Vice District Governor, Cabinet Secretary, Cabinet Treasurer and the Immediate Past District Governor shall be ineligible for this award.
- The Multiple District Four Excellence Award medals will be made available through the Multiple District Four office. The award will consist of a medallion to be hung from a presentation ribbon and a resolution of appreciation issued by the Council of Governors.

CATEGORIES
Greatest Number of New Clubs by a District Governor
Districts wishing to participate will be required to submit their list of Chartered Clubs formed
during the period July 1 thru June 30.
Greatest Number of New Clubs by a Member
Districts wishing to participate will be required to submit their list of Extension Award
Recipients (or list of Chartered Clubs with name of the Lion who formed the Club) during the period July 1 thru
June 30.
Greatest Number of New Members by a Member
Districts wishing to participate will be required to submit the name of the Lion within their
respective Districts who was credited with sponsoring and/or reinstating the most members during the period
July 1 thru June 30.
Most Significant Service Project of the Year
This award is designed to feature a Club’s project that is hands-on in nature and for which members
turn out to do physical labor or service for the betterment of someone in their community. It should be
the embodiment of our motto, 'We Serve'. The amount of money spent will NOT be considered and the project
should not be considered a club fundraiser. Districts wishing to participate should nominate projects done
during the time period July 1 thru June 30. Previous Academy Award winners in this category
are not eligible.
Most Outstanding Project of the Year
This award is designed to feature a Club’s 'signature' project; the one for which the Club is most
renowned. The amount of money spent may be considered and the project could be considered a Club
fundraiser. Districts wishing to participate should nominate projects done during the period July 1
thru June 30. Previous Academy Award winners of the Most Original and Unusual Service Project are
not eligible.
Lion of the Year for the Multiple District
Districts wishing to participate should submit their District Lion of the Year for the period July
1 thru June 30. Consideration for this award will include service at district level or above
during the given time period. All Lions in good standing with MD-4 are eligible. The current DG and VDG
will not be eligible.
Lioness of the Year for the Multiple District
Districts wishing to participate should submit their District Lioness of the Year for the period
July 1 thru June 30. Consideration for this award will include service at district level or
above during the given time period. All Lionesses in good standing with MD-4 will be eligible.
Leo of the Year for the Multiple District
Districts wishing to participate should submit their District Leo of the Year for the period July
1 thru June 30. All Leos in good standing with MD-4 will be eligible.
The Kay and Denise Fukushima Couple of the Year Award
A couple selected by their District Governor will have shown exemplary leadership on behalf of the
District and Multiple District, based on the theme, 'One World, One Heart'. Service through the years
should be considered, not just the current year. A couple for this award would include a Lion and a
significant other who does not necessarily have to be a Lion.
The Donald E. Banker Campership Award
The Lion selected by his or her District Governor will have demonstrated extraordinary effort,
either over many years or during the current one, in enhancing the success of one of our youth camps. This
would include any camps that the Multiple District or any sub-district maintain and directly support.
SCRAPBOOK CONTEST
RULES
The Vice District Governors, in attendance, shall judge the Scrapbook Contest at the first Council meeting after the International Convention. The following rules for judging shall be followed:
- Only winners of the individual District level contest shall be eligible. District’s or Club President’s scrapbook or yearbook may be entered in the contest.
- The President’s year shall begin with his/her election and will end on June 30th the following year.
- There will be three awards for the MD-4 contest: a winner and two honorable mentions. Awards will be presented at the MD-4 Convention the next February.
- Maximum Cover size of book shall be: 19" x 25"
- Lions, not professionals, shall have prepared book.
- The scrapbook will be divided and judged in eight categories:
- Organization and index (7 pts); general appearance (3 pts) 10 pts
- Local community service 20 points
- District service activities 15 points
- MD4 activities 10 points
- International activities 10 points
- Youth activities 10 points
- News media coverage 15 points
- Club social activities 10 points
